New Student Required Registration Documents

Required Registration Documents

To apply for school, please include the following documents with your application. 

  1. Completed registration form
  2. Birth Certificate
  3. Immunization Records (records must be up to date, with month and year of each vaccine dose given, signed or stamped by clinic, physician or nurse) 
  4. Parent/Guardian Driver’s License or ID 
  5. Two (2) Proof of Residency (in parent/guardian’s name) 
     • Mortgage Statement, Lease Agreement, Tax Bills, Gov't mail
     • Utility bill 
  6. Legal Documents (if applicable: custody orders, adoption records, guardianship documents, legal name change orders, restraining orders, etc) 
  7. IEP/504 Plan (if applicable)
